1. The owner completing this form (the "registrant") acknowledges and agrees that the information concerning this registration will be maintained on a data base PETinfo(the "Data Base") operated or controlled by EIE KOREA / ELECTRONIC INFORMATION EQUIPMENT KOREA Co. (the "Company") provided however that the Company reserves the right to have such services performed by a third party at the company's cost. The registrant agrees that the registration on the Data Base and the information set out therein is the exclusive property of the Company and it shall be entitled to utilize such information as it deems fit.

2. The initial data base registration fee covers all of the registrant's animals for the lifetime of the registrant. A service charge will be assessed for each addition and/or modification to the data base after 30 days. The service charge payment must accompany the data base addition and/or modification form.

3. The registrant acknowledges and agrees that the service provided by the Company is a registration and information service only. The Company does not guarantee or warrant the accuracy or veracity of the information on such registration its Data Base and only obliged to provide such information to the registrant or any party authorized by the Company to obtain such information in accordance with the terms for use thereof as specified by the Company from time to time.

4. The registrant agrees that it is his/her sole responsibility to ensure that the information as set out on the registration is accurate and is kept up to date. The Company is only obliged to have the registration input to the Data Base within 30 days of its receipt of the completed registration form or within thirty days of receipt of any notice of change of the information set out therein. The registrant assumes full responsibility for the said registration form being received by the Company notwithstanding any acts or conveniences provided by the Company. within 30 days of receipt of a print out of the registration, the registrant must advise the Company by certified or registered mail of any errors or inaccuracies therein. Unless such notification is received by the Company, as aforesaid, the information shall be deemed to be correct. In the event that the information in connection with the registration changes then it is the sole responsibility of the registrant to notify the Company of such change within thirty days of such change occurring by written notification sent by certified or registered mail to the Company and shall at such time pay the Company's then processing fee. the Company shall have thirty days from the date of receipt of such change(s) notification to input same. Unless the registrant has received notification that the change has been made, same will be deemed not to have been made and it shall be the sole responsibility of the registrant to pursue such matter. The registrant acknowledges that the Company does not and will not verify any information set out in the registration.

5. The registrant agrees that this agreement is personal to the registrant and in the event of a sale or transfer of the animal which is the subject of the registration must be changed to the new owner's name by the registrant. Such notification shall be sent to the Company by certified or registered mail within 30 days of such transfer being completed and at such time shall also pay the Company's fees to change such registration at the Company's then fee therefor. The Company shall not be obliged to input such changes unless it has received the fee therefor in advance.

6. The registrant agrees that once the registrant's animal has been implanted with the Company's implantation identification devices, that it will not tamper with or impair same. In the event that the registrant notice any rejection or impairment of the implantation, he or she shall forthwith advise the party who performed such implantation of the same in writing with a copy to the Company failing which the registrant shall assume all responsibility for any effect, loss and damage such rejection or impairment may cause.

7. The registrant acknowledges and agrees that the service provided by the Company is an information service only and the Company shall not be responsible for any loss, damage, claim, or liability whatsoever in connection with same or the registration maintained in connection therewith except where same was caused by gross negligence on the part of the Company as confirmed by a written communication from the Company containing information which is different from the information recorded on the Data Base. In the registrant agrees that in the event the information is utilized inaccurately or negligently by any party to whom same is delivered by the Company, the registrant shall look only to such other party and the Company shall not be liable for any loss, cost, claim, damage or liability whatsoever in connection with same. The registrant acknowledges that the use of such information is separate and distinct from the maintenance of the Data Base or system.

8. The registrant acknowledges and agrees that any veterinarian, humane society, association or any other party authorized by the Company to perform implants or to participate in the Company's system are independent from the Company are not partners of or agents for the Company.
